I take full responsibility for my individual experience and outcome associated with this 1st Annual Gathering of the Medicine Women Retreat. I understand that Bird Mejia, Art of Balance LLC, Susan Lana, Andre Douglas, Malikah Hugs and Deborah Redshirt are not doctors or psychologists and what they share in no way substitutes for professional care. I agree that I will seek professional and medical advice from my doctor or mental health professional as to any changes in mental health treatment, medication, or anything related. Under no circumstances will Bird Mejia, Art of Balance LLC or Susan Lana, Andre Douglas, Malikah Hugs and Deborah Redshirt be held responsible for my actions or circumstances. My heirs, guardians, legal representatives and l hereby and forever release, waive, and discharge any claims against Bird Mejia, Art of Balance LLC or Susan Lana, Andre Douglas, Malikah Hugs and Deborah Redshirt are, and any of their associates, affiliates, or family. Under No Circumstances Shall Art of Balance, LLC or Bird Mejia, or Susan Lana, Andre Douglas, Malikah Hugs and Deborah Redshirt be Liable for Any Damages that Result from the Use of or Inability To Use the Services of Bird Mejia, Art of Balance LLC or Susan Lana, Andre Douglas, Malikah Hugs and Deborah Redshirt are. User Hereby Acknowledges that this paragraph shall apply to all content, courses, and services available through Bird Mejia, Art of Balance, LLC or Susan Lana, Andre Douglas, Malikah Hugs and Deborah Redshirt.
I acknowledge that the 1st Annual Gathering of the Medicine Women Retreat does not allow for cancellations or refunds of any kind. I recognize that the space which I have agreed to occupy is a highly sought after space. Should I decide at any point that I no longer wish to participate in the 1st Annual Gathering of the Medicine Women Retreat, I do so with the complete awareness that I will not receive compensation of any sort or a refund in any amount. If I have chosen the payment option, I agree to complete all payment installments as scheduled.
I have carefully and thoroughly read and understand this agreement. I am aware that by checking this box, I am agreeing to and I am legally bound to the aforementioned statements.
